Community Health Workers (CHWs) serve as a crucial link between individuals and essential services, addressing housing instability as a key factor in overall well-being. At La Familia Sana, our CHWs specialize in housing assistance, helping individuals and families navigate complex systems to secure stable living conditions.​​
What Our CHWs Do:

Housing Assistance & Navigation – Helping individuals apply for housing programs, understand eligibility, and connect with landlords or housing agencies.

Advocacy & Case Management – Supporting clients throughout the housing process, providing follow-ups, and coordinating with other agencies.

Access to Healthcare & Social Services – Connecting individuals to Medi-Cal benefits, mental health services, and other essential resources.

Community Outreach & Education – Identifying those at risk of homelessness and providing information on available housing solutions.

Who Qualifies for CHW Services?
CHW services are available to anyone with Medi-Cal or a participating managed care plan. Thanks to California's CalAIM initiative, non-profit organizations like La Familia Sana can now provide housing assistance through CHW services, recognizing the connection between housing stability and health.

This program is covered through Medi-Cal Partnership Insurance, which enables La Familia Sana to serve more individuals efficiently and expand access to critical housing support.
